Privacy policy

In order to provide services to you as a client, we need to collect, record and use your personal information relevant to your engagement with Haven Occupational Therapy. This may include but is not limited to, medical information, contact details and potentially sensitive information about you or your child.

Any information gathered as part of your assessment or intervention process will be kept securely, and in the interest of your privacy will only be used by your therapist and other relevant Haven Occupational Therapy personnel (i.e. admin staff, OTA’s etc). Your personal information is retained to track your progress, assist your therapist to provide relevant recommendations and informed therapeutic decision making. 

You are entitled to access your personal information as recorded in Haven Occupational Therapy at any time unless relevant legislation states otherwise. If this is something that you require, please highlight this to your therapist and we will organise the data sharing process with  you. 

With the above information in mind, by signing our service agreement you give consent to the following:

  • Store information about me and/or my child. 

  • Haven Occupational Therapy to collect mine and/or my child’s personal information in order for them to provide services to me and/or my child in the form of paperwork, photo, video or audio records.

  • Relevant and Haven Occupational Therapy personnel to access information about me and/or my child to provide services to me or my child.

  • OT students can attend, observe, participate in, plan and lead sessions under the direct supervision of a qualified occupational therapist. 

  • Cannot share information with the following person/s: 

  • Use appropriate telehealth platforms and recognise that these might not always be 100% secure 

  • Share information with other relevant stakeholders.
